Medical Questionnaire

ALL senior players (1s to 3s) will need to fill out the Medical Questionnaire form here and bring it to the club for one of the physios (Sophie and Charlotte) to review. If no form is received the player will not be able to play a league match.

The forms once signed and returned to the physios will be reviewed to see if they need to ask further questions then will be stored in a folder in the physio room under lock and key. Only Sophie and Charlotte will be reading them.
